Leeway PILOTS Housing Taking Applications

Dear Community Provider:

Leeway is pleased to announce that we are currently taking applications for
our PILOTS housing site located at 153 Bull Hill Lane, West Haven, CT
06516.  This unit is a one bedroom occupancy.

As one of our community providers, we are requesting you assist all eligible
applicants in completing the attached application. This is a CHFA property.
Individuals must meet the eligibility criteria of being chronically homeless or at
risk of homelessness and also must be diagnosed with prolonged mental
illness and/or chronic substance abuse. The applicants will have to clear a
criminal background check. Please read the information carefully and return
the completed application as well as the requested documentation to:
