During the 100 Day Campaign, we have started developing
relationships with many community organizations that make it easier for our
clients to obtain identification. We want to make sure all case managers
in New Haven are benefiting from these relationships. Specifically, we
have created working relationships with Social Security Administration and the
DMV.
A Social Security representative
will be at the Columbus House Annex (592 Ella T. Grasso Blvd.) on July 15, 2014 from 1 PM to 3 PM completing
applications for new social security cards. We will send around a sign up email closer to 7/15
to have your clients access this service. Documentation needed to
obtain a Social Security card is medical records, birth certificate, OR photo
ID. Please let Veronica Cruz and/or Jennifer Nulty know if you are sending a client over.
Every Wednesday and Friday morning at 7:45 am, the DMV will be holding windows for us. If you would like to join us in the morning,
please let Veronica Cruz and me know. Your client must come with all documents
ready and the fee if needed. To obtain a duplicate State ID, the clients
need: 30$, 2 pieces of mail, and an original copy of a Social Security card OR
a birth certificate. To obtain a first time State ID, the client needs:
DMV fee waiver, 2 pieces of mail, an original copy of both Social Security card
AND a birth certificate.
